- Coverage will end the date you leave the organization.
- In most cases, coverage can be continued under COBRA for up to 18 months.
- You will receive COBRA election notice mailed to your last known home address from our COBRA administrator, Ameriflex, containing election instructions.
- Flexible Spending Accounts (FSA):
- You have 90 days from your last day of employment to submit eligible claims for reimbursement. Claims must be submitted for services prior to your last day of employment.
- Health Care FSAs can continue if your Health Care FSA is under COBRA. Dependent Care FSA participation ends on your last day of work.
- Disability and Life Insurance:
- Short-term and Long-term Disability ends on your last day of work.
- Basic and Optional Life and Accidental Death and Dismemberment (AD&D) Insurance ends on your last day of work. Don’t miss your chance to keep this insurance (not disability coverage). COBRA continuation coverage
Under certain circumstances, after you leave VyStar or you no longer meet benefits-eligibility requirements, you and your dependents may continue to participate in some VyStar benefits plans through COBRA continuation coverage. If you have a COBRA qualifying event, you’ll receive information regarding your specific coverage rights.
